- You can only fill out the survey once per month.
- Fill out the survey within 30 days of your purchase of boots
- You will not get any advantage of a boots card.

Rewards:

- A winner will be selected by random draw.
- Can’t exchange the prize for cash or anything else.
- After winning the prize you should contact Boots Retail within 28 days otherwise, they do a random draw for an alternative winner.
- End of the survey you should provide your email address so Boots will contact you by your email.
- You will get your prize by post.
